Athletic Clearance

Online Athletic Clearance Instructions (2025–2026 School Year)

 

To participate in school sports, all student-athletes must complete the online clearance process. Please follow the steps below carefully.

 

Step 1: Visit the Athletic Clearance Website

• Go to www.AthleticClearance.com and select Florida as your state.

• OR go directly to the Florida login page: https://athleticclearance.fhsaahome.org

 

Step 2: Watch the Tutorial (Optional)

Step 3: Parent Registration

• Parents/Guardians must create an account using a valid email address and password.

• Important: Save this login information — you will use it every year your student participates in athletics.

 

Step 4: Begin a New Clearance

• Click “Start Clearance Here”

• Choose the correct school year: 2025–2026

• Select all sports your student will try out for or participate in.

 

Step 5: Complete Required Sections

• Fill out:

  • Student Information

  • Educational History

  • Medical History

  • Signature Forms

• If you’ve completed a clearance in previous years, you can select existing student and parent/guardian profiles from the dropdown menus.

 

Step 6: Upload Required Documents

• Upload all pages of the following forms:

  • EL2 – Physical Evaluation Form (must be completed by a healthcare provider; valid for one calendar year)

  • EL3 – Consent and Release of Liability Form

  • CRF-1 – Code of Conduct Form

• Also upload:

  • Proof of Medical Insurance

  • Birth Certificate

  • File formats accepted: PDF or JPEG only.

 

Step 7: Add More Sports (Optional)

• Want to register for additional sports or activities?

  • You can select more options after the confirmation message.

  • Electronic signatures will apply to all selected sports.

 

Step 8: Submission & Clearance

• All submitted documents go directly to the school’s athletic department for review.

• You will receive an email notification when your student is cleared to participate.

 

If denied, you’ll also receive an email with the reason for denial.

 

Athletes may not participate until officially CLEARED in the system.

 

No Paper Copies Accepted

 

Absolutely NO physical documents will be accepted by coaches or administrators.

 

All forms must be submitted online through the clearance platform.
